Moved FETCH_BUILD_NUMBER to front of driver.makefile so that it is useable by USR_DBFLAGS in module makefiles

No one is using cvs anymore

Added mechanism for installing breakpoint tables and menus and other .dbd files separate from a main module dbd file

Added 'prebuild' target for users to run before build; this can be added to a module.makefile to allow certain tasks to be run before the build process occurs.
