CE-2124: replace/remove dialogs with common

• removes/replaces custom SimpleModal and ConfirmationModal with ce-ui-common components
• removes/replaces LoginDialog and LoginForm components with ce-ui-common components (turns out we already don't use LoginDialog, and LoginForm already exists in ce-ui-common but we weren't using it!)

Affects all dialogs, e.g.:

• login/logout
• IOC Details:
  • Manage:
    • updeploy/deploy
    • start/stop
  • Admin:
    • Modify IOC
    • Change Host
• Administratively undeploy
• Delete
• Log Details / Host Details:
  • console/loki popout
