Pv names

Anders Lindh Olsson requested to merge pv_names into master

Modify iocsh and template files to provide easier interface for various modules (see example.st.cmd for usage) as standard configuration concept no longer appears to be followed at ESS (card X in slot Y).

Modify database (substitution and template) files for PV names to comply with ESS Naming Convention.

Fix minor bugs in MSI linkage.

General clean-up of repository (.MD files and logs).

Note that $E3_MODULE_VERSION so far has been left unchanged - change hingent on when build for e3 share will happen. Module/application is not in use right now. Build number should be appended if required.
