Use IOCNAME environment variable for require PV names

Wayne Lewis requested to merge waynelewis/require:ICSHWI-4525 into master

This makes the PV names persistent across IOC reboots. The original use of the REQUIRE_IOC environment variable resulted in PV names that would change on each IOC restart, and would also change if the host changed.
